To become a member of the East Gwillimbury Public Library a registration form must be completed in full and signed.  Current identification providing name, permanent address and phone number must be presented:

Adult (18 + years of age)

Driver’s license, Employee identification, Utility bill, personal cheque or any other source of information that gives the patron’s name and current address (preferably photo ID)

Youth (14 – 17 years of age)

Driver’s License, Bill (cell phone), Report card with address, Pay stub or Direct Deposit stub, Bank statement, Magazine subscription, Student identification card

Child (0 – 13 years of age)

Parent’s identification and Signature of parent on registration form
